Resumen
Paracoccidioidomycosis (PCM) is a systemic mycosis caused by Paracoccidioides spp., common in men and rare in immunocompetent women due to the protective effect of estrogen. We present the case of a 28-year-old woman from Huancayo, previously diagnosed with PCM, who discontinued antifungal treatment. Five months later, she developed painful cervical masses, axillary and inguinal lymphadenopathy, and skin lesions. Grocott and PAS stains reconfirmed PCM, associated with microcytic anemia and Helicobacter pylori infection. She received amphotericin B deoxycholate, which was complicated by severe hypokalemia and gastrointestinal symptoms, requiring adjustments and antibiotic management. She recovered favorably and was discharged on an outpatient basis. This case highlights an unusual presentation in an immunocompetent woman and the need for timely diagnosis and treatment adherence.
